When filling out the form:
The group name is "Bayshore Christian School Reunion."
Person responsible for billing: Your name & info.
On Site Contact: Jennifer Funkhouser
Check the middle box on the form, which says "I authorize charging $_(fill in the total cost for attendance: 1 person $45, 2 $90, etc.)_ for a non-refundable deposit to my credit card."
Then fax the form at the # given, or e-mail it to jmanthey@yachtstarship.com. If you cannot copy and send
